Social Connection Program

Social Connections is a FREE person-centred program that pairs those from all walks of life with a friendly Peer Connector for their individual support.

A Peer Connector is someone who will listen to your social needs and ask questions about your interests, hobbies and struggles to help develop a plan and provide you with the right support to connect you to your community. Once you have a plan, the Peer Connector supports you to get started and continue.

What is the Social Connection Program?

Being a part of the Social Connections Program will provide you with the individual support to help you engage into community based upon your interests, goals, and social needs, build your self-confidence and empower self-management skills.   3. A Peer Connector will be in contact to have a chat
  4. We will create a plan together to connect you to community
  5. We will review your plan to see if Social Connections is working for you
