Are you offering support as a healthcare professional?

The Social Connections Program is based a Social Prescribing model which enables healthcare professionals, social and frontline workers to refer patients or clients to a Peer Connector. It’s the entry point for lonely and socially isolated individuals in accessing a person-centred focused program aimed at increasing social connectedness. Healthcare professionals play a key role in supporting patients to access non-medical services that support social needs.

The aim is to share knowledge and inform best practices and support social prescribing programs like Social Connections to reach the most vulnerable and isolated people.

If you believe your patient may benefit from being a part of our Social Connection services, you can make a referral.

Key benefits The Social Connections Program

Our Social Connections Program pairs individuals with a Peer Connector for free, who provides them with the support to help engage into community based upon their interests, goals and social needs.

  • Offering a unique opportunity for health professionals, researchers, community representatives, service providers, community members, voluntary sectors and funders to work together to assist and expand community and voluntary activities.
  • Provides an integrated approach to help people counteract issues of loneliness and social isolation and connect to fun, supportive community activities, programs or events.

Who is Eligible?

  • Adults aged 18 years plus

  • Live in the Ballarat Goldfields or Grampians Wimmera Region

  • New in town, feel lonely, socially isolated or just want to become more socially connected.

  • Wanting to improve their sense of community connectedness and belonging

How to Make a Referral

We have endeavoured to make the referral process as easy as possible. There are a couple of ways to refer a patient or client into The Social Connections Program. Please note, that we require you to have your patient’s or clients consent before providing information on their behalf.
