Humans are social beings; and social interactions are a core psychological need, essential to our overall health and wellbeing. Our social needs are often fulfilled by socially connecting with others and community initiatives and organisations, groups or clubs play a vital role in making individuals in their community feel connected to where they live.

Community services, organisations, groups and clubs play a crucial role in social prescribing models like The Social Connections Program.

Community Mapping

Our role at Social Connections is to link lonely or socially isolated individuals into community initiatives and services that support their social needs. The only way we can achieve this is by knowing of what services, activities, programs and groups are operating across the community.

The aim is to leverage the already existing community services to facilitate greater social connection for our community members.

We’ve tried to deliver a resource where an organisation can search for services that might help your clients with complex health needs, but also have space where organisations like yours can be accessed by individuals themselves and/or health professionals.

Get involved

By informing us about what your organisation offers, we can connect our clients to your services and or programs.

  • Contact the Social Connections Team to have your organisations programs or services listed on the Website
  • Refer a client that might need to access Social Connections Program.
  • Or, Take the pledge to building better connected communities to reduce the issues of social isolation and loneliness
